All portions of this form must be completed and any supporting documentation must be submitted; incomplete forms will not be processed. If the participant pool coordinator confirms you did complete the study, you will receive full credit. NOTE: If you did not show up or you were late for the experiment, it is at the researcher's discretion to determine whether your no show stands or will be changed to zero credits. If there are any questions regarding this claim, the coordinator will contact you for further information. Please allow up to one week for any corrections or changes to appear online.
