Undergraduate Forms

Plans of Study

Majors

Complete three (3) copies of the Plan of Study form and bring them to your Psychology faculty advisor for approval within the first four (4) weeks of the semester in which you are graduating. Once they are signed by your advisor please bring them to the Undergraduate Studies Office in BOUS 100A for Dr. Miller's signature.

    • Minors


    Complete two (2) copies of the Plan of Study form and bring them to the Undergraduate Studies Office in BOUS 100A within the first four (4) weeks of the semester in which you are graduating.

  • Psychology Minor to be signed by Dr. David Miller, Associate Dept. Head of Psychology.
  • Neuroscience Minor to be signed by Dr. John Salamone.
  • Criminal Justice Minor to be signed by Greg Champion (in BOUS 100).

    Undergraduate Research and Independent Study


    When enrolling in PSYC 3889 (Undergraduate Research) or PSYC 3899 (Independent Study) you must complete the two forms listed below. Please obtain signatures from your lab supervisor and Psychology advisor on both forms before bringing them to BOUS 100A for Dr. Miller's signature.

  • Undergraduate Research Information Form
  • Independent Study Authorization Form

    Field Experience


    Students must make arrangements for PSYC 3880 (Field Experience) with their faculty sponsor BEFORE the start date of the field experience. When enrolling in PSYC 3880 you must first complete the Field Experience Learning Plan and obtain the signatures of your field and faculty sponsors. You must also complete the Independent Study Authorization form and obtain the signatures of your faculty sponsor and Psychology advisor. Once completed please bring both forms to BOUS 100A for Dr. Miller's signature.
